Dr. Jennifer Hohman of Van Wert Pediatrics was presented with the Immunization Coverage Improvement Award by the Ohio Department of Health. This award is presented to healthcare providers that achieve significant improvement in immunization coverage rates, as indicated by results of the most recent Assessment, Feedback, Incentives, and Exchange (AFIX) continuous quality improvement process.

AFIX is a continuous quality improvement process that requires the commitment and dedication of providers to achieve high levels of immunization coverage for the recommended series of vaccines.

“I applaud your practice’s work to achieve excellence in immunization practices; it is through these efforts that Ohio is working toward the goal of 90 percent immunization coverage of patients by 24 months of age,” said Theodore E. Wymyslo, M.D., director of the Ohio Department of Health. “Please take the opportunity to share your immunization strategies with fellow providers so that Ohio may continue to keep children protected against vaccine-preventable diseases.”
