Community asked to take health survey

As part of the ongoing Community Health Improvement Plan process, members of the local community are being asked to complete a Quality of Life Assessment.  The easiest way to complete the assessment is at the following link: https://www.surveymonkey.com/s/FJ5LHTF.

Community members completing the survey are also being asked to encourage as many other community members as they can to do the same. The survey must be completed by next Thursday, October 24.
