Centerville woman killed in head-on crash

The Ohio State Highway Patrol’s Van Wert Post is investigating a fatal crash that occurred at 12:20 p.m. Monday on U.S. 127, near Walnut Grove Church Road in York Township, Van Wert County. The crash was approximately six miles south of the city of Van Wert.

According to investigating troopers, a 2012 Freightliner semi-tractor-trailer rig driven by Ben A. Bernard, 69, of Zanesville, was southbound on U.S. 127. A 2002 Buick driven by Carole B. Dunaway, 76, of Centerville, was northbound on U.S. 127. Dunaway crossed the centerline into the path of the Freightliner and struck it head-on.
Dunaway was pronounced dead on scene by Van Wert County Coroner Dr. Scott Jarvis and she was transported to the Lucas County Coroner’s Office by Brickner Ambulance Service. The driver of the Freightliner was transported to Van Wert County Hospital by EMS and treated for minor injuries.
The Highway Patrol was assisted on scene by Ohio City Volunteer Fire Department and EMS, Brickner EMS, Van Wert County Coroner and Van Wert Sheriff’s Office.
The crash is still under investigation. Alcohol is not believed to be a factor in the crash. Seatbelts were in use.
