Main St., Convoy<\/li>\n<li>J&amp;J Butler Sales, 200 Walcott St., Willshire<\/li>\n<li>Van Wert Veterinary (Dr. Trapp) on Van Wert-Decatur Road<\/li>\n<li>The Ramblers Roost on Lincoln Highway, near Middle Point<\/li>\n<li>Van Wert Animal Clinic (Dr. Wilkin) on Lincoln Highway east of The Marsh Foundation<\/li>\n<li>Ohio City Express, 511 W. Carmean St., Ohio City<\/li>\n<li>Brenda\u2019s Cuddles &amp; Cuts, 1333 N. Main St., Delphos<\/li>\n<li>Emme Lu\u2019s Pet Wellness Center LLC, 706 W. Ervin Road, Suite C, Van Wert<\/li>\n<\/ul>\n<p>A self-addressed, stamped envelope, as well as the proper fee, must accompany the return of a license application by mail. If purchasing a tag at one of the above locations, take the application, if one was received by mail.<\/p>\n<p>Cost of a dog license is $16 per tag. A penalty of $16 per tag, in addition to the license cost and fee, must be paid for licenses purchased for dogs 3 months of age or older after January 31, 2012.<\/p>\n<p>Kennel licenses are $80 for five tags. Extra tags are $1 each.
