This award is presented to healthcare providers that achieve significant improvement in immunization coverage rates, as indicated by results of the most recent Assessment, Feedback, Incentives, and Exchange (AFIX) continuous quality improvement process.<\/p>\n<p>AFIX is a continuous quality improvement process that requires the commitment and dedication of providers to achieve high levels of immunization coverage for the recommended series of vaccines.<\/p>\n<p>\u201cI applaud your practice\u2019s work to achieve excellence in immunization practices; it is through these efforts that Ohio is working toward the goal of 90 percent immunization coverage of patients by 24 months of age,\u201d said Theodore E.
