Registration will be conducted during the week of March 2-6 from 7:30 a.m. to 4 p.m. at Lincolnview Elementary School.<\/p>\n<p>Registration will require about 10 minutes. Registration forms are also available on the Lincolnview website at <a href=\"http:\/\/www.lincolnview.k12.oh.us\">www.lincolnview.k12.oh.us<\/a>. Parents may complete the form and bring it with them to registration to expedite the registration process. They will be given a kindergarten screening appointment at the kindergarten registration. Those who want their children to attend kindergarten at Lincolnview, but live outside the district, should plan to register and attend Lincolnview\u2019s screening.\u00a0 Open enrollment space can be limited. Parents can obtain an open enrollment form at registration for the kindergarten class. Applications are considered in the order received. The open enrollment window is April 1 to May 31 for all other grade level students.<\/p>\n<p>Parents should bring their child\u2019s Social Security card, official birth certificate, and immunization records<b> <\/b>to registration. Include custody papers, if applicable.<\/p>\n<p>Immunizations needed before starting school include<b> <\/b>polio, four doses; DPT, five doses; MMR, two doses; Hepatitis B, three doses; varicella (chickenpox), two doses.<\/p>\n<p>Lincolnview Kindergarten Screening will be held Tuesday, April 7, and Wednesday, April 8, at First Baptist Church on Jennings Road.
