Main St., Convoy.<\/li>\n<li>The Ramblers Roost Truck Stop, Lincoln Highway, Middle Point.<\/li>\n<li>Animal Clinic-Dr. Wilkin, Lincoln Highway, Van Wert.<\/li>\n<li>Van Wert Veterinary Clinic-Dr. Trapp, Van Wert-Decatur Road, Van Wert.<\/li>\n<li>Ohio City Express, 511 W. Carmean St., Ohio City.<\/li>\n<li>Delphos Ace Hardware, 242 N. Main St., Delphos.<\/li>\n<li>Emme Lu\u2019s Pet Wellness Center LLC, 7079 U.S. 127, Van Wert.<\/li>\n<li>Dog House Grooming &amp; Supplies, 117 N. Washington St., Van Wert.<\/li>\n<li>J &amp; A Merchandising, 200 Walcott St., Willshire.<\/li>\n<\/ul>\n<p>A self-addressed envelope and the proper fee must accompany the return of the application by mail.\u00a0 If purchasing a tag at one of the above locations please bring the license application if one was received.<\/p>\n<p>Dog tags may be ordered online from December 1-January 31 only.
